- Who files it: The property owner, or an agent such as a contractor, signs it.
- When you need it: Any time work needs a construction permit in NJ, before work starts.
- How many copies: one original plus three copies, per DCA. Your town may ask for more.

Field-by-field help
- Owner legal name
- Use the owner's full legal name, as shown on the deed.
- Owner mailing address
- Use the address where the owner receives mail.
- Owner phone
- Use a phone where the office can reach the owner.
- Owner email
- Use an email the owner checks.
- Owner date of birth
- Only provide a birth date if a form needs it. Saving it needs your consent. Example: 1980-01-15
- Owner lives at the property
- Say whether the owner lives at this property. Do not assume this grants an exemption. Example: true
- Property street address
- Use the actual work site address. A mailing city does not prove the governing town.
- Governing municipality
- Confirm which town governs the property. A postal city alone is not proof. Example: Edison Township
- County
- Confirm the county that contains this property. Example: Middlesex
- State
- Use the state where the work will take place. Example: NJ
- Tax block
- Find the block on your tax bill, survey or tax map. Do not guess. Example: 101
- Tax lot
- Find the lot on your tax bill, survey or tax map. Do not guess. Example: 12
- Tax qualifier
- Copy a qualifier only if your tax record lists one. Example: C0001
- Zoning district
- Ask the zoning office for the parcel's district. Do not use a town boundary as parcel zoning. Example: Confirm with zoning office
